Those lines are not from a play...

...or any other literary work: they're Shakespeare's epitaoh. More precisely, they are lines carved on a stone near his grave in Trinity Church, Stratford-Upon-Avon, and which are thought to refer to him, or to have been written by him, although neither of these points is certain.
