Yes it was Susanna

Susanna apparently got caught up in the crackdown in the wake of the Gunpowder Plot. She was on the list at Stratford for failing to make Anglican communion on Easter in 1606. She also apparently ignored a church summons to answer the charges or explain herself. The charges were marked dismissed. (Bribe to the vicar?) It's also interesting that Hammnet and Judith Sadler (Judith Shakespeare's godparents) appear on the same list. I believe the Sadlers are also mentioned in Shakespeare's will.
