A Speculation...

Regarding the appearance of Caesar's ghost:

You are correct, the second appearance of the ghost
is mysteriously "skipped" over. This play, more than any other,
seems to be the play where Shakespeare toys with that
magin number "three."

This is pure speculation, but I have noticed that this
"second" appearance is followed (technically) by a third. The
instant before Brutus dies, he sees it yet again (as I recall
his dialogue). Now, might the Bard have been arranging things
dramatically so that this would make for "three" appearances,
in keeping with all of the other occurrences of that
"special, magical" number?
